Understanding vitiligo

What it is, and what we look at

In vitiligo, melanocytes — the cells that give skin its colour — are destroyed, most likely by an autoimmune process. The result is milky-white patches with clear borders, often first appearing on the hands, face, around the mouth and eyes, or in areas of friction. It carries no physical danger, but the social and emotional impact in India is considerable and frequently underestimated.

Treatment goals are realistic: to slow or halt further spread, and to encourage repigmentation where follicles remain active. Areas rich in hair follicles, such as the face, tend to respond better than the fingertips and lips. Response is slow and measured over many months.

Symmetrical distribution in the common form
New patches appearing at sites of injury or friction

What tends to be behind it

  • Autoimmune destruction of melanocytes
  • Genetic predisposition and family history
  • Association with thyroid disease and other autoimmune conditions
  • Skin trauma, cuts, burns and friction at affected sites
  • Chemical exposure such as certain phenolic compounds

When to see a doctor promptly

Homeopathic treatment is not the right first step for these. Seek medical assessment if:

  • Patches are spreading noticeably over weeks
  • Patches appear around the eyes, or vision changes occur
  • There is accompanying fatigue, weight change or hair loss in patches
