Understanding alopecia areata

What it is, and what we look at

Alopecia areata typically begins as one or more coin-sized bald patches on the scalp or beard, with skin that looks otherwise normal. It is autoimmune in origin, not caused by hair products or washing. Hair often regrows on its own, sometimes white at first, but the condition can also relapse.

It is associated with other autoimmune conditions such as thyroid disease and vitiligo, so screening is sometimes appropriate. The psychological effect can be substantial, particularly when patches are visible, and that is taken as seriously as the skin finding itself.

You may recognise these

Sudden, smooth, round patches of hair loss
Patches on the scalp, beard, eyebrows or eyelashes
Short broken hairs at the edge of a patch
Skin within the patch that appears normal, not scaly
Pitting or ridging of the nails
New patches appearing while others regrow

  • Autoimmune attack on the hair follicle
  • Genetic predisposition and family history
  • Association with thyroid disease, vitiligo and other autoimmune conditions
  • Severe physical or emotional stress as a reported trigger
  • Viral illness preceding onset in some cases

When to see a doctor promptly

Homeopathic treatment is not the right first step for these. Seek medical assessment if:

  • Patches spread rapidly or merge into large areas
  • Eyebrow, eyelash or body hair is also lost
  • There is accompanying fatigue, weight change or skin depigmentation

In limited patchy alopecia areata, spontaneous regrowth within a year is common. Extensive or long-standing disease is less predictable. Neither outcome can be promised in an individual case.
