What it is, and what we look at

Vertigo is a symptom, not a diagnosis. The commonest cause is benign paroxysmal positional vertigo (BPPV), where brief intense spinning is triggered by head position changes such as rolling over in bed or looking up. Other causes include vestibular neuritis after a viral illness, Ménière's disease with hearing loss and ear fullness, and vestibular migraine.

Distinguishing these matters, because BPPV responds to specific repositioning manoeuvres rather than to medication. Case taking establishes the duration of each episode, what provokes it, and whether hearing, headache or neurological symptoms accompany it.

When to see a doctor promptly

  • Vertigo comes with weakness, slurred speech, double vision or facial droop — seek emergency care
  • There is sudden hearing loss in one ear
  • There is severe headache, or vertigo follows a head injury

No, though they are often confused. Vertigo is a spinning sensation; low blood pressure or anaemia usually causes light-headedness on standing. Describing which one you experience helps considerably in narrowing the cause.
