Understanding varicose veins

What it is, and what we look at

Veins in the legs carry blood upward against gravity, helped by one-way valves and the pumping action of calf muscles. When the valves weaken, blood pools and the veins stretch and become visible, twisted and raised. Symptoms typically build through the day — heaviness, aching, swelling around the ankles — and ease on elevating the legs.

Beyond appearance, long-standing venous insufficiency can cause skin discolouration, eczema around the ankle and eventually ulcers that heal poorly. Compression stockings have good evidence for symptom relief and are usually the first measure. Case taking covers occupation, standing hours, pregnancy history and any skin change at the ankle.

When to see a doctor promptly

  • A vein becomes hot, hard, red and painful, or the calf swells suddenly — seek urgent assessment
  • The skin near the ankle breaks down into an ulcer, or is discoloured and thickened
  • A varicose vein bleeds — lie down, elevate the leg and apply firm pressure, then get medical help
