Understanding childhood growth concerns

What it is, and what we look at

A single height or weight reading means very little on its own. What matters is the trajectory — whether a child is tracking steadily along their growth curve or has crossed downward through centile lines over time. Many children who look small are constitutionally small or are late developers with a family pattern, and are entirely healthy.

Genuine growth failure needs assessment, because treatable causes exist: nutritional deficiency, coeliac disease, hypothyroidism, chronic infection, recurrent illness and, less commonly, growth hormone deficiency. Case taking covers birth weight, feeding, diet, illness frequency, parental heights and prior growth records, with referral where a specific cause is suspected.

Height or weight noticeably below peers of the same age
Crossing downward through centile lines on the growth chart
Clothes and shoe sizes unchanged for a long period
Poor appetite or very selective eating
Frequent illness with slow recovery
Delayed puberty compared with peers

Causes & triggers

What tends to be behind it

  • Familial short stature or constitutional delay in growth
  • Inadequate calorie or protein intake
  • Iron, vitamin D and other micronutrient deficiency
  • Hypothyroidism and, less commonly, growth hormone deficiency
  • Coeliac disease, chronic infection and other long-term illness

When to see a doctor promptly

Homeopathic treatment is not the right first step for these. Seek medical assessment if:

  • Growth has crossed downward through two centile lines
  • There is weight loss, chronic diarrhoea or persistent fever
  • There is no sign of puberty by 13 in girls or 14 in boys

Not necessarily. Children of short parents are usually short, and some are simply late developers who catch up. What warrants attention is a child who was tracking one centile and has drifted downward across lines, or who has stopped growing.
