Understanding chronic fatigue

What it is, and what we look at

Long-standing fatigue is one of the most common complaints in general practice, and a substantial proportion has an identifiable and correctable cause: iron deficiency anaemia, hypothyroidism, vitamin D or B12 deficiency, diabetes, sleep apnoea, depression, or medication side effects. Testing for these is the first and most useful step, and they are frequently missed when fatigue is dismissed as stress.

Where investigation is unremarkable and fatigue persists beyond six months with unrefreshing sleep, difficulty concentrating and a characteristic worsening after exertion, myalgic encephalomyelitis or chronic fatigue syndrome is considered. Notably, pushing through with graded exercise can worsen this condition, so activity pacing is the approach used rather than progressive exercise.

When to see a doctor promptly

Homeopathic treatment is not the right first step for these. Seek medical assessment if:

  • There is unintended weight loss, night sweats, or persistent fever
  • There is breathlessness, chest pain or enlarged glands
  • Fatigue comes with persistent low mood or thoughts of self-harm

Common questions

Asked before the first visit

Full blood count and ferritin, thyroid function, blood glucose or HbA1c, vitamin D, vitamin B12, and kidney and liver function. Coeliac screening and a sleep apnoea assessment are added where the history points that way. These identify most correctable causes.
