Understanding back pain

What it is, and what we look at

The large majority of low back pain is non-specific and mechanical — related to posture, muscle strain, prolonged sitting or lifting — and settles over days to weeks. Imaging in the first few weeks generally does not help, because disc bulges and degenerative changes appear on scans of many people with no pain at all, and finding them can lead to unnecessary worry and intervention.

The exceptions matter. Pain radiating below the knee with numbness or weakness, pain after significant trauma, or pain with fever, weight loss or bladder and bowel changes all need prompt assessment. Case taking screens for these first, then addresses the mechanical pattern, posture and workstation setup.

Pain worsening with prolonged sitting, standing or bending
Muscle spasm and restricted movement
Pain radiating into the buttock or thigh
Numbness, tingling or weakness in the leg
Stiffness that is worse in the morning

What tends to be behind it

  • Muscle or ligament strain from lifting or sudden movement
  • Prolonged sitting and poor workstation ergonomics
  • Disc degeneration or prolapse
  • Weak core and gluteal muscles
  • Excess weight, and in older adults, osteoporosis

When to see a doctor promptly

Homeopathic treatment is not the right first step for these. Seek medical assessment if:

  • There is loss of bladder or bowel control, or numbness around the groin — seek emergency care
  • There is progressive leg weakness, or pain after a fall or accident
  • Back pain comes with fever, unintended weight loss or a history of cancer

Asked before the first visit

Usually not in the first six weeks of uncomplicated back pain. Scans frequently show disc bulges and degenerative changes in people with no symptoms, and early imaging is associated with more intervention without better outcomes. Red-flag features change this — those warrant imaging promptly.
