What it is, and what we look at

PCOS is diagnosed when at least two of three features are present: irregular or absent ovulation, clinical or biochemical signs of excess androgens such as acne and excess facial hair, and polycystic-appearing ovaries on ultrasound. Importantly, ovaries that look polycystic on a scan are not by themselves a diagnosis — many women have them without PCOS.

Insulin resistance is present in a large proportion of women with PCOS, including those who are not overweight, and it links the condition to longer-term risks of type 2 diabetes. That is why treatment addresses metabolism as well as the cycle. Case taking covers cycle length, weight history, skin and hair changes, family history of diabetes, and fertility plans.

When to see a doctor promptly

  • You have not had a period for three months or longer
  • Bleeding is unusually heavy or prolonged
  • You have rapid hair growth with voice deepening — this needs prompt assessment

No. PCOS is a common cause of difficulty conceiving because ovulation is irregular, but many women with PCOS conceive, with or without help. If you are trying to conceive, say so early, since it changes the treatment plan and the timeline.
