What it is, and what we look at
Gout occurs when uric acid levels are high enough for crystals to form within a joint. An attack begins abruptly, often at night, with intense pain, redness, swelling and heat — typically in the base of the big toe, though ankles, knees and other joints are also affected. The pain is characteristically severe enough that even bedsheet contact is unbearable.
Attacks settle within days, which leads many people to treat each episode and then stop. That is the central mistake: without lowering uric acid between attacks, they recur, and over years crystals accumulate and damage joints. Long-term management is more important than acute relief, and kidney function should be checked, as gout and kidney disease are linked.

When to see a doctor promptly

Common questions
Asked before the first visit
Diet helps but is rarely sufficient on its own — it typically lowers uric acid by a modest amount. For recurrent attacks or tophi, urate-lowering medication is the established treatment, and it is taken continuously rather than only during attacks.
