What it is, and what we look at
Gastro-oesophageal reflux disease develops when the valve between the stomach and the oesophagus does not close effectively. Acid then irritates the oesophageal lining, producing heartburn, a sour taste, and sometimes throat symptoms — chronic cough, hoarseness and throat clearing — that are frequently mistaken for a throat problem.
Long-standing untreated reflux can cause changes in the oesophageal lining, so persistent symptoms deserve proper assessment rather than indefinite self-medication. Case taking covers timing relative to meals, night symptoms, dietary pattern, weight, smoking and any use of NSAIDs.

When to see a doctor promptly

- There is difficulty or pain on swallowing, or food sticking
- There is vomiting of blood, black stool, or unintended weight loss
- Chest pain radiates to the arm or jaw, or comes with sweating — treat as cardiac until proven otherwise

Asked before the first visit
Yes, several have solid evidence: eating the last meal at least three hours before lying down, raising the head of the bed by about 15 centimetres, losing excess weight, and stopping smoking. These often do more than adding another tablet.
